That's odd.  It's completely clear to me which one needs to come off in order to keep the shoulder functioning...  Doesn't matter if the smaller one in the front functions better, it's not attached to anything but his chest...

I'd say leave them all, too.  Although, you have to guarantee the family that his lot in life won't be "circus freak".</description>
